Non-gatefold cover with printed banana, "THE VELVET UNDERGROUND & NICO" printed on top left. Back cover with credits on top, no Emerson. Some came in misprinted "white banana" cover. Something went wrong during the printing and the yellow banana became almost white. The printing company stamp which is on lower right back cover is "SPU" on the misprinted issue while the regular "yellow" printing reads "DJ".

Back cover has credits and mentions cassette release catalog number. No barcode. Also reads below the classic portraits "MANUFACTURED BY POLYGRAM INC. AND DISTRIBUTED BY POLYGRAM DISTRIBUTION INC., 6000 COTE DE LIESSE, ST-LAURENT, QUEBEC H4T 1E3 - MADE IN CANADA".
